Claims Documentation

     The following documents are needed to process your claim:

 

  1. Notice(s) of Claim to Carrier and all responses received from the Carrier.

     
  2. Packing List showing the quantity and value of items shipped. IF International shipment valued at over $ 250 USD: Also provide a copy of the Commercial Invoice, which should show the cost of each shipment, the terms of the invoice and the total value of the shipment. A Pro Forma Invoice will not be accepted in lieu of a Commercial Invoice.
     
  3. IF Missing Shipment (non-delivery) - a written, signed and dated Pickup Receipt, Manifest, or Bill of Lading from the Carrier must be provided.  In the case of parcel carriers or Postal Service, delivery status records documenting physical receipt of package by reported intransit or sortation status will be accepted in lieu of signed pickup documents.  Note: Shipper manifest reports and system uploads do not confirm carrier receipt and will not be accepted as proof of carrier receipt.
    IF Missing Items (aka "short-shipments/partial delivery") - a
    Delivery Receipt from the Carrier showing actual quantity delivered must be provided. An Over Short or Damage Report may be accepted in lieu of the Delivery Receipt, if duly completed, dated and signed by an identifiable individual who received the shipment.
    IF Damaged Goods – a
    Delivery Receipt with noted exceptions for the damage must be provided.

     
  4. Photographs Whenever and wherever possible, all physically damaged shipments should be photographed. This also includes the exterior of shipping cartons that indicate possible tampering (such as cut tapes or shrink-wrap) or the interior of the carton showing condition of goods at the time of receipt.

     
  5. Repair Invoice(s) for damaged but repairable items.

     
  6. Replacement Invoice(s) for damaged but un-repairable items. Please note that certain items may require an independent evaluation and confirmation of the value of the item.
     

 

Promptly upload, fax, or email copies of the above documents.  The promptness, thoroughness, and legibility of these documents directly affect the speed of claims settlement.
